Athlete Health Forms

Several health and medical forms must be filled out before an athlete can participate with a Coffeyville Community College athletic activity.  The four forms below need to be completed, submitted and on file with the CCC athletic training department by August 1, 2008.  Forms can either be delivered in person or mailed to:

• 2008-2009 Policies and Procedures for Drug Testing (Only page 5 needs to sent back to Coffeyville Community College).

The two documents below do not need to be returned to CCC, but should be printed and kept for your information.
