Athlete Health Forms
Prior to participating on a team from Coffeyville Community College, athletes must provide the Athletic Department with current address, emergency contact, insurance, medical alert, copy of insurance card and health history information. To expedite this process Coffeyville uses an online data entry system. This will also be used by the coaching staffs to receive information in regards to ability to play of an injured athlete. The two documents below will assist in navigating this web portal.

Medical Evaluation Form
CCC also requires a Medical Evaluation Form prior to participation on a team. The form below can be printed and filled out by your physician and submitted to the Athletic Training Department upon your arrival to CCC.
