Dear Student: 

Thank you for your interest in our Health Programs.  The CNA and CMA courses will be based on the first 10 students that send in their Entrance Form and payment.  Additional classes will be scheduled if the class exceeds the maximum student capacity. 

Below is a list of materials needed to be mailed in with your entrance form.  You will be added to the class roster when we receive the following corresponding documents:

Certified Nurses Aide

• Completed entrance form.

• Payment or documentation of payment by an agency.

• Copy of social security card.

• Additional information at Kansas Department of Health and Environment

Certified Medication Aide

• Completed entrance form.

• Payment or documentation of payment by an agency.

• Copy of CNA State Card.

• Additional information at Kansas Department of Health and Environment

You must be a current Certified Nurses Aide to be eligible for enrollment.  A copy of your current CNA State Card must be submitted with the CMA state application before the state will issue your CMA Card.

CMA Re-certification

• Completed entrance form.

• Payment of documentation of payment by an agency.

• Copy of CMA State Card.
